Job Description:
Lee Health is seeking a dedicated RN Case Manager for our Home
Health Central team in Southwest Florida, focused on providing
personalized patient care in a home setting. This role involves
coordinating patient care, managing treatment plans, and ensuring
the highest quality of care is maintained. Ideal for a
compassionate professional skilled in multitasking and patient
communication. Assess patients' health problems and needs in their
homes Develop and implement nursing care plans for patients
Coordinate with healthcare providers and agencies to provide
appropriate care Monitor and report changes in patient symptoms or
behavior Educate patients and their families on proper home health
care strategies and procedures Strong clinical skills in nursing
Excellent communication and interpersonal skills Ability to work
independently and as part of a team Strong organizational and time
management skills Proficiency in electronic health records (EHR)
systems Medical Insurance Dental Insurance Vision Insurance Life
Insurance 401k Current Registered Nurse (RN) license in the state
of Florida CPR certified Case Management Certification (CMC)
