Job OpportunityBayCare HomeCare is seeking a dedicated and compassionate Home Health RN to join our team. As a Home Health RN, you will provide high-quality patient care in the comfort of their own homes, utilizing the nursing process and clinical standards to ensure optimal outcomes.Responsibilities:

• Lead the multi-disciplinary team providing care to patients
• Monitor the provision of therapeutic services through written and verbal communication with all disciplines
• Ensure all visits scheduled and provided are authorized and covered by physician orders
• Review patient visits weekly and provide a plan to the Patient Care Supervisor and team assistant
• Inform the scheduler of any schedule changes and communicate caseload and patient care issues to the Home Care SupervisorRequirements:
  • BLS (Basic Life Support) certification
  • RN (Registered Nurse) license
