Your Specific Responsibilities:

The incumbent is part of a global public health organization and is expected to support the Florida Department of Health in Polk County in its mission to protect, promote, and improve the health of all people in Florida through integrated state, county and community efforts and its vision to be the Healthiest State in the Nation. Furthermore, the incumbent is expected to incorporate the agency values of Innovation, Collaboration, Accountability, Responsiveness and Excellence in all aspects of their work.

Primary Duties - Acts as New Patient Intake Coordinator, for Test & Treat (HIV+), PrEP (Non-HIV+ patients), returning to care HIV+ patients:

* This position will work with HIV infected patients and other patients who are at risk of HIV, or those who have or are at risk of Sexually Transmitted Diseases. As the lead "Intake" staff, this position will conduct psychosocial risk assessments, and assessments of all incoming new or returning to care HIV infected clinical patients, and other patients seeking services which are provided at the Specialty Care clinic, such as STD Testing and Treatment and providing services related to Pre-Exposure Prophylaxis for HIV. This includes but is not limited to testing, scheduling, entering information in the HMS Medical Record System, and tracking patient information in various systems and spreadsheets.


* Works closely with Disease Intervention Specialists in the STD Unit in order to begin HIV treatment services for newly diagnosed patients and with the Linkage to Care Coordinator to re-establish HIV Patients who have been lost to care.


* Conducts initial phone interviews with all new HIV infected patients wanting clinical services. Assessing all medical needs and giving appropriate and timely appointments for new HIV infected client services.


* Conducts history and initial client assessments on all new and returning HIV infected clients.


* Acts as liaison between clinical services and case management to ensure continuity of care through coordination and follow-up of medical treatments.


* Exchanges information with other health and/or rehabilitative agencies to provide continuity of care for HIV infected clients receiving mental health services from more than one agency.


* Makes appropriate referrals for HIV infected clients who need long-term mental health counseling.


* Provides referrals and information to patients for short-term mental health counseling for HIV infected clients while establishing referral linkage.


* Participates in regular multi-disciplinary team meetings.


* Works together with other multi-disciplinary team members to assess, plan, develop interventions for HIV infected client's high-risk needs. Also, works to evaluate the high-risk HIV infected client's care plan for outcomes and re-assess this plan as needed.


* Utilizes RN STI Protocol for treatment of Sexually Transmitted Infections as appropriate.



Secondary Duties - as needed and as time permits - Clinical Duties:

* Works in clinic.


* Preparation of the clinic area and equipment.


* Assist physicians and ARNPs with exams and procedures.


* Interprets, educates, and reinforces the provider's plan of care for the client.


* Interview clients to obtain complete health history and presenting complaints.


* Take vital signs and perform other screening tests.


* Collect specimens for laboratory tests and interpret laboratory results.


* Follow up on abnormal and late lab reports.


* Administer medications as ordered by physicians or under medical protocol and guidelines.


* Make appropriate referrals and assist clients to make necessary arrangements.


* Maintains accurate records and reports.


* Assess immunization status of all clients and immunizes in accordance to current standards.


* Enters immunization information into Florida Shots.


* Provides screening for the HIV antibody virus, pre and post test counsels, provides education, and refers for medical evaluation and other support services.


* Document and maintain accurate data entry into the client Electronic Health Record.



Other Functions:

* Participates in staff and in-service educational programs.
* Attends assigned committee meetings.


* Assists with inventory of supplies, medications and vaccines.


* Maintains and increases competency through educational materials and classes.


* Assists in orientation of new personnel and volunteers.


* Assists with supervision, as a charge nurse, of the clinical staff and the clinic as assigned by the Sr. CHN Supervisor.


* As a state employee of the PCHD - employee will participate in Disaster Preparedness and Recovery Activities.


* This position may serve clients that speak only Spanish, Creole, or other languages. It may require performance of


* interpreter duties, if bilingual, or implementation of the Department's limited English proficiency plan.


* Performs other duties as required, including performance of additional interpreter duties, consistent with skill level, and as needed.

This occupation requires licensure as a Registered Professional Nurse in accordance with Chapter 464, Florida Statutes or eligible to practice nursing in accordance with Florida Administrative Code 210-8.22 or 8.27 or Certification as a Physician's Assistant in accordance with Chapter 458, Florida Statutes.
